YOUR ADMISSION

Patients being admitted to the San Day Surgery Hornsby are requested to read the following information.

Complete all Admission forms given to you by your doctor and return them to the San Day Surgery Hornsby at least 3 working days prior to admission. These can be hand delivered, posted, or faxed. (fax number 9476 2921) Please bring any original documents with you on the day of your surgery.

The staff at the San Day Surgery Hornsby will contact you between 3pm and 6pm the afternoon prior to your scheduled surgery to confirm hospital admission time, fasting time and any other preparations necessary for your surgery. You will be advised at that time if there are any additional payments required.
