Your Admission

Your admissions forms must be received at least 3 days prior to your procedure at San Day Surgery Hornsby. Forms can be completed online or manually.

Online admissionsTo allow us to process your admission promptly, please complete your forms online through our eAdmissions Patient Portal.


In most cases, your doctor will have given you a Pre Admission Booklet when you booked your admission. Please read this booklet carefully and retain it for your information. If you complete your admission online you DO NOT have to fill in the admission and patient history forms in the booklet manually.

If you did not receive this booklet you can download our Pre - Admission Booklet here, which includes the following forms:

  • Hospital Booking Letter - completed with your doctor
  • Consent to Medical or Surgical Treatment Form - completed with your doctor
  • Admission Form - can be completed online
  • Patient History Form - can be completed online

If you choose to complete the forms manually you must deliver them to San Day Surgery Hornsby at least 3 working days prior to admission. These can be hand delivered or posted to the address below, or faxed to 02 9476 2921. Please bring any original documents with you on the day of your surgery.

The staff at the San Day Surgery Hornsby will contact you between 3pm and 6pm the working day prior to your scheduled surgery to confirm hospital admission time, fasting time and any other preparations necessary for your surgery. You will be advised at that time if there are any additional payments required.

Valuables

Please do not bring excessive cash and/or valuables with you. While all care of valuables is taken, the San Day Surgery Hornsby does not accept liability for lost or damaged personal items or valuables.
